corner of 128th and Gibson in EverettHit-and-run accidents are some of the worst kinds of crimes because the driver generally doesn’t even have the courtesy of calling 911, but instead only thinks selfishly as their victim bleeds, and often dies, alone on the side of a road. That may have been what happened early Sunday morning.

Snohomish County sheriff’s deputies were called to the 1100 block of 128th Street SW, near E. Gibson Road, about 12:30 am where they found the man on the side of the road, dead.

A witness told detectives he saw a dark-colored pickup truck leaving the scene.

The witness’s description matched the description of a vehicle involved in another hit-and-run that was reported earlier on Saturday evening outside a tavern at 148th Street SW and Highway 99 near Lynnwood. Nobody was hurt in that incident.

Police found the pickup’s owner, but he told them the truck had been stolen. Detectives believe the owner was not involved in either incidents.

Investigators are going to look at footage from traffic cameras at an intersection near the scene of the fatal hit-and-run. Police say they are looking for a dark-blue 2005 Ford Ranger with a Washington license plate B35457F. If you see that truck, call 911.

The victim and her husbandThere is a major misconception in society that there is an epidemic of random crime, but when statistics are studied by experts, they find crime is in fact not random and can be concentrated and at times even predicted. But it doesn’t mean that some crimes aren’t random and that may have been the circumstance in the death of a beloved Tacoma government worker.

It all started with an altercation over a text between two teenage girls about 11:30 pm last night. A group of young people gathered in-front of the house next door to Lisa Melancon, a codes enforcement officer for the Safe Streets Campaign for the City of Tacoma.

Amie Hieronymus, 18, had been living for only a week next door to the Melancon home in the 7200 block of South Bell Street. She was somehow involved or knew people involved in the altercation.

Hieronymus said she went to the Melancon home seeking help when the fight outside got out of control.

The victim’s husband, Joe, went outside to see what was going on while the victim called 911, she said.

Shortly thereafter, Melancon came to the door to call her husband in, members of one group got into a vehicle and started driving off. As they did, one occupant started shooting “apparently kind of randomly,” Tacoma police spokesman Mark Fulghum said.

Hieronymus said a bullet whizzed past her head and struck Melancon.

Melancon was struck twice in the face killing her instantly.

“When I looked up, Lisa was on the ground,” she said. “That bullet was supposed to be for me. I feel so guilty. I should have never knocked on her door.”

A description of the suspects and their vehicle has not been released. No arrests have been reported, but Hieronymus said she knows one of the men involved. Tacoma Police are questioning suspects, but have not made any arrests as of yet.

“We got some information and leads,” Fulghum said.

Tacoma-Pierce County Crime Stoppers is offering up to $1,000 for information leading to arrests and charges in the homicide. Tipsters may remain anonymous and can reach Crime Stoppers at 253-591-5959.

Melancon, the oldest of three siblings, moved to Tacoma about 16 years ago from her native Sacramento seeking a better life for her and her now 19 year-old son, her brother Vinnie Silva said.

She went to work for the city of Tacoma, which she grew to love, Silva said.

She worked for the city for 15 years in various community outreach programs and married her husband about 7 years ago.

“She was unselfish with her time, her resources, her love,” Silva said. “She let people’s good reflect. That’s how she remembered people: for their good traits.”

This murder is a result of many factors colliding in the wider American society such as gun laws, local enforcement, municipal policy, and educational funding. It is not isolated to Tacoma and can, in fact, be found in cities small and large across the nation. Certain laws and policies are changing regarding youth crime and as a result youth crime has gone down. Tacoma is a good example of this. Unfortunately there is a long way to go and the death of Melancon is a prime example of that. It is just too bad that she had to be victim to the crusade with which she was a part.

Michael Kekoa RavenelWhen the state pays out, we the tax payers pay out. But when the system falters, it is up to all of us to make sure that things are made right again in order to retain a culture of accountability through fair punishment and just compensation. The sentiment was carried-on on Friday when in the wrongful death case of Michael Kekoa Ravenell, a bubbly, smiley 3-year old.

Family members noticed that bad things happened whenever Noah Thomas, the mother’s boyfriend, was around.

The boy’s father, Michael Ravenell, said he would find bruises and cuts on his young son and he immediately reported it to Child Protective Services (CPS), a division of Washington State Department of Social and Health Services (DSHS) dedicated to protecting the well being of those who cannot protect themselves.

Unfortunately, the system failed him, “Every time I tried to call or report a new incident, I didn’t get any feedback,” he said.

The wrongful death lawsuit against the state said Kekoa’s aunt and grandmother also tried to warn CPS about the apparent abuse. But the state neglected to follow up or run a background check on Thomas, so CPS officials had no idea that he’d been convicted of abusing his own children previously and posed a serious threat to the little boy.

In the weeks before Kekoa’s death, his father called Child Protective Services and reported seeing a bruise near the boy’s left eye. Ravenell also noticed changes in the behavior of the boy and his sister.

Just in two months of reporting the abuse, Kekoa was beaten and strangled to death by Thomas, who ultimately admitted hurling Kekoa onto a bed, where his head hit an iron rail.

Then Thomas took a nap while the boy died.

“I didn’t want to believe it. I was telling him to wake up,” says his heartbroken father. “He didn’t wake up.”

Kekoa’s younger sister Chelsea was also a victim of Thomas. She was 1 1/2 years old at the time and suffered a broken jaw by his hand.

Thomas, now 27, pleaded guilty to homicide by abuse and was sentenced last year to 50 years in prison.

In response to the death of his son, Kekoa’s father filed a wrongful death lawsuit against the state last year, alleging social workers didn’t fully investigate the allegations of child abuse before the boy’s death. The suit also contended DSHS hadn’t protected Kekoa’s sister, who also was abused. Doctors discovered she had a broken jaw that had been healing for some time after Kekoa’s death.

A state review of the boy’s death found that the social worker assigned to his case had too little experience and missed several steps while investigating the bruising reported by the boy’s father. The social worker involved in Kekoa’s case was fired after his death. Since then, the state says that when an abuse referral comes in, caseworkers evaluate anyone involved in a child’s life.

The state says the $2.1 million settlement fairly compensates the family and that they’ve made significant changes.

“We are really paying more deliberate attention to any adult who has access to the children, even if the person does not live there every day,” Revels Robinson, of DSHS, said. “That was a very critical lesson learned here.”

But it was too late for Kekoa.

“I was hoping that CPS would do the right thing,” Michael Ravenell says.

The lesson here is change. So many abuse cases come from those outside of the family and it is fortunate that now Kekoa’s legacy can save the lives of so many children. Over 100 children have died since 2002 while having some kind of contact with the state. Perhaps now that number can be lowered considerably.

Soap LakeSometimes bright sunny days and dry road conditions give drivers a false sense of security when driving. Sure, the roads aren’t slick and visibility is good, but many times the heat off of the pavement can obscure debris in the road. Also, the conditions could inspire drivers to drive faster than they normally would which may be what happened near Soap Lake, WA yesterday afternoon.

Three teenagers were riding in a car headed westbound on State Route 28, 6 miles east of Soap Lake about 4:15 pm when the driver lost control of the vehicle and slammed into a power pole.

The Washington State Patrol says two of the passengers were pronounced dead at the scene, the third person was taken to a hospital with serious injuries.

Crash Investigators were still at the scene of the collision Thursday evening. Troopers say the names of the victims will be released pending notification of next of kin.

The road was partially blocked as investigators processed evidence and utility crews removed power lines that were draped across a few lanes.

Update: We are sorry to announce that the injured teen died today of traumatic head injuries.

Though speed may not have been a factor in this crash, it could have very well have been an animal running out into the road or various other hazards, speeding is the largest cause of death in men 15-35. It can result in the victim spending days if not months in the hospital recovering from serious personal injuries followed by a long period of physical therapy. It is the first minutes and hours after the crash that are the most critical though. That is when victims are most vulnerable, but it is particularly affecting when the patient is in the care of professionals and stabilized just to slip away days later as was the case with a Tacoma woman who died this week at Tacoma General.

The crash happened around 3:15 in the afternoon on Monday in the 500 block of Sixth Avenue, a Tacoma police spokesman said. Gertrude Lewin stepped into the street from between the two parked cars and was hit by an SUV.

The driver immediately called 911 and medical crews rushed to the scene. They were able to stabilize her and get her to hospital. She was able to hold on through the night and part of the day on Tuesday, but unfortunately died of her injuries.

Airplane Crash in Ferry CountySmall plane crashes are caused by mechanical error and pilot error almost equally according to statistical websites that track causes and outcomes. On top of that, small plane crashes are also most prevalent both at take-off and landing. Both of these statistics may come to play as a plane crash this morning in Ferry County is investigated.

According to the Colville Indian Tribal Police Chief, one man is dead and another suffered serious personal injuries after a plane crashed along Highway 21 near Keller, WA around 6:45 am this morning.  Initial speculation is that the small two-person plane took off on a private runway and crashed just after takeoff.  The plane was found tangled in power lines along the side of Highway 21.

Both victims were conscious when emergency crews arrived but the pilot died shortly after.

Tribal police did not release the names of the victims but did say the two victims are both men and they believe both of them are 65 years old.  The victims are not related.  One of the men is from the Ferry County area and the other is not according to police.

The FAA is currently investigating the accident.

Update: The deceased’s name is Elwynn Fletcher, 65, according to Colville Tribal Police. Fletcher was a long-time cattleman in the Mesa, AZ area and was well-known across Washington state in the cattle industry. Justin Marchand, 24, of Colfax was also in the plane and was flown to a Spokane hospital, where he is listed in serious condition.

Infant TylenolJohnson & Johnson (J&J) and their subsidiary McNeil PPC (the manufacturer of numerous Tylenol products including children’s products) have been feeling the heat this year. They were forced to recall millions of units of their Tylenol, Benadryl, and Pediacare products due to foreign particles, bacteria contamination, and worst of all, inconsistent dosage.

The Food and Drug Administration (FDA) has since begun a thorough investigation that, in the end, may reveal severe sanitation problems, serious quality control issues, and there has already been speculation of a cover-up at the executive level regarding the reporting of problems to the federal health agencies.

All of this came months prior to yesterday’s verdict, perhaps the first in the nation, regarding serious problems with inconsistent dosage levels of over the counter Children’s Tylenol. Children’s medicine is very specific because a larger than prescribed dose of medicine can cause serious liver damage and death in small children who are already vulnerable to illness. This is the case with Louisiana couple regarding their infant daughter who lost her life through a series of hospital misdiagnosis and drug company negligence.

Wrongful Death Lawsuit

It was January 3, 2003 when the parents of Brianna Hutto took their infant to Opelousas General Health System’s (OGHS) emergency room. She was diagnosed with a fever and cold and her parents were instructed to give her “Infant’s Tylenol.” 5 days later, on January 8th, the 5 month old died of liver failure.

The lawsuit names OGHS and McNeil-PPC, the company that makes Infant’s Tylenol, as to blame for Brianna’s death. It says, the hospital and company misinformed the family on the proper dosage amount of the medicine.

An Opelousas jury agreed by awarding the parents $7.5 million.

“They were devastated, as you can imagine. I mean they were 18 at the time, they just lost their 5 month old baby,” Hutto’s attorney said. “Their whole life was taken from them. Words can’t describe… they have felt that way continuously, the pain has not gone away.”

The Hospital has released a statement voicing their regret over the situation and touting their dedication to quality care. Neither J&J nor McNeil have given a statement regarding the verdict, but their recall of their products says a lot about the danger of the products.

The Hutto’s lawyer said that he hopes it stays off of shelves for good. “I think the most important part of this story is not necessarily the amount of money that was awarded, but the fact that Infant’s Tylenol is a dangerous product.”

Perhaps it’s the thrill or just mere impatience, but either way, when there are other people in the car, it is the driver’s responsibility to drive defensively. Unfortunately, one young man didn’t follow the rules, and a seemingly innocent thrill turned into a fatal truck accident that killed one of his friends and today sent him to jail.

Derrick A. Laiblin, the victim, and two other friends were passengers in a pickup truck driven by Jacob Lee playing a cat-and-mouse chase with another car of friends when he entered the on-coming lane to pass and lost control about 5 pm Aug. 29.

They crashed in the 28800 block of the Sumner-Buckley Highway. The road had a no-passing, double yellow line in that area, and Lee’s silver-colored Ford F350 had been going 60 to 70 mph in the 35 mph zone, according to witness reports.

Laiblin died at the scene. The two other friends suffered broken arms and serious personal injuries.

In a teary eyed court proceeding, Lee, 19, was sentenced to three years and five months in prison for one count of vehicular homicide and two counts of vehicular assault.

After sentencing, Lee got up and said in a shaky voice, “I want to say sorry and that my heart goes out to all three of those families, I want to say sorry to my family … I’m ready to take responsibility for it and come out a better person.”

This is an unfortunate story of some young men who were horsing around resulting in very serious consequences that they didn’t calculate prior to doing it. Deadly crashThere are too many crashes on the road due to fatigued driving. A person may feel awake and then the next thing they know they are waking up in the hospital with serious personal injuries. Unfortunately for one couple, they won’t wake up at all after a nice long weekend of camping with family and friends with a horrific end.

The accident occurred in the westbound lanes near Milepost 34 just before 7 am. The couple, a 30 year-old man and a 26 year-old woman were apparently returning from a camping trip in eastern Washington with their 4 year-old son and two family friends, a 15 year-old male and a 20 year-old male.

Washington State Patrol said the car, a 2005 Dodge Neon, drifted off to the left shoulder where it hit a bridge abutment and guardrail and “blew apart.”

“We’re dealing with such a high impact into this bridge abutment, striking this guardrail, that there’s really nothing that could have saved them,” said WSP. ”I believe they were seatbelted, but this is one of the cases that when you hit so hard that nothing’s going to help you out.”

The couple in the front seat were killed instantly. The teenager was rushed to the hospital with in serious personal injuries. The 20-year-old is said to be fine. The couple’s four-year-old boy seemed to have received only minor injuries.

All three were taken to Harborview Medical Center in Seattle for treatment and observation.

“[The four-year-old] was actually strapped into a car seat, which we have at the scene, and it looks to us that that absolutely saved his life,” said a WSP trooper.

It is hard to predict if you are showing signs of fatigued driving, but it is best to be cautious. Washington Car Accident Attorneys: Local Florist Killed Crossing Hwy 904 in Cheney

After a pedestrian has been hit by a car there are those who may blame the pedestrian for their own death merely because they decided to wear dark pants or decided to cross outside of the crosswalk. We do not yet know the details of last night’s tragedy, but the circumstances don’t take away from the grief being felt by some in the Cheney community.

James Dahl, 61, owner of Chet’s Flowers on Highway 904 in Cheney was crossing the street in front of his business. At the same time 47-year-old William Knight was driving westbound on State Route 904 when he hit as Dahl was crossing.

Dahl was still alive after the crash and during transport to the hospital, but succumbed to his injuries at Sacred Heart Medical Center around 2:30 Monday morning.

Knight, and his passenger, were both wearing seatbelts and were not seriously injured. The Washington State Patrol says there haven’t been any charges filed against Knight at this time, but the crash is under investigation.

Troopers say drugs or alcohol were not involved.

Update – It was revealed by KXLY TV that this wasn’t the first time Knight hit someone while driving. On April 15 bicyclist Ted Chauvin says Knight hit him while he was riding his bike several miles outside of Cheney as reported on Phillips Webster Blog at the time of the accident. Chauvin suffered a broken leg, arm and missed five weeks of work after the crash. Also, apparently Knight had no insurance at the time. Authorities are investigating and said an arrest may be pending.
